Brief Description: The Bridge Project assists young people living with FASD make the transition from adolescence to adulthood a positive healthy experience. The project will be flexible and creative in delivering its services and will be individualized, strengths based, relationship focused and client centered in providing services to 36 young people over the three years. Services will include housing, individualized case planning, coordinated case management, assistance with activities of daily living, practical support to access medical, mental health and addiction services; the opportunity to develop life skills, access employment training suited to skills, assistance in securing and maintaining employment, practical support to families and caregivers, and advocacy with the larger service system. Overall goal of the project is to meet individual needs of young people with FASD while working with the larger service community to find ways to address the issues of access to adult services and access to supported housing.
