Brief Description: The purpose of this study is to follow-up with mothers and mother/child dyads discharged from the Fir Square Combined Care Unit at BC Women’s Hospital in order to identify key health, housing, custody and other social issues faced by mothers in the 3 month period following discharge. The study will also identify the services that mothers’ access over the 3 months immediately following giving birth and needed post-intervention supports for this population. It is hypothesized that access to safe housing and being able to return to the community with custody of their infant will contribute to positive outcomes for high risk, substance-using mothers who have received specialized in-hospital maternity care. The findings will be used to inform Fir Square on the effectiveness of current discharge planning practices and the Ministries of Health Services and Child and Family Development on the impact of service system policies and gaps.
